The nonprofit San Francisco Campus for Jewish Living serves a significant number of patients and long-term care/skilled nursing residents each year in its Jewish Home & Rehab Center. Non-residential care is offered through our short-term and rehabilitation services unit for patients who require specialized interdisciplinary rehabilitation to recover from surgery or an acute illness, and through our acute geriatric psychiatry hospital. With a complement of close to 650 compassionate, experienced clinical and allied health personnel, SFCJL provides older adults with a variety of life-enriching healthcare programs and services in a welcoming, stimulating environment.

SFCJL’s transformed campus includes the new Lynne & Roy M. Frank Residences (assisted living apartments and memory care & support assisted living suites), and Byer Square, a non-residential community of programs, services, and support.

POSITION OVERVIEW:

The Admissions Nurse Liaison is responsible for clinically assessing potential patients for admission to the San Francisco Campus for Jewish Living’s (SFCJL) short-term and long-term programs. The position works closely with acute-care hospitals, health systems, discharge planners, and case managers to insure a safe transition between care settings for new patients and residents. The Admissions Nurse Liaison is the clinical lead and acts as a liaison between the Admissions team and the Home’s nursing function.

The Admissions Nurse Liaison will work closely with the Director of Admissions, playing an integral role in helping to achieve the organization’s short-term census goals and long-term growth and revenue objectives.

ESSENTIAL FUNCTIONS:

  • Assess and review patient referrals, from a multitude of sources, to determine clinical appropriateness for potential admission into the Campus. Identify clinical needs/requirements for admission and communicate to Nursing team.
  • Verify and interpret potential patients’/residents’ medical insurance, determining their coverage for a skilled nursing admission.
  • Explain and articulate SFCJL’s services to both internal and external audiences, focused on converting referrals into admissions.
  • Act as a liaison with Nursing staff to connect the two departments (Admissions and Nursing), and their respective functions.
  • As a member of the Admissions team, assist in the management of census and patient/resident ‘throughput’ – with a focus on meeting/exceeding stated census goals and targets.
  • Identify and qualify potential referral sources for patient admissions, typically from hospital-based referral sources. Promote growth of admissions.
  • Establish and maintain significant professional relationships with referral sources, physicians and other healthcare professionals to increase referrals.
  • Represent JHSF in a professional manner the community.
  • Performs other duties as assigned.

QUALIFICATIONS:

  • Valid California LVN/RN license preferred or equivalent work experience.
  • Minimum of 2 years of nursing or clinical experience required.
  • Prior healthcare, Discharge Planning, or case management experience, preferred.
  • Knowledge of the Medicare rules for skilled nursing preferred
  • Demonstrated proficiency with MS Office, including Outlook, Word and Excel.
  • Demonstrated high level of interpersonal skills to handle sensitive and confidential situations.
  • Demonstrated ability to build rapport quickly with peers, referral sources, patients and families.
  • Demonstrated ability to develop and build strong, lasting relationships with referral sources, clients and families, preferred.
  • Strong communications skills, both written and verbal.
  • Excellent planning, organization and presentation skills are critical.
