This includes the Frank Residences, offering luxurious assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.

Position Overview:
Reporting to the Health and Wellness Director and the Memory Care Director, the Care Coordinator supports the entire care team by role-modeling and reinforcing best practices for care delivery in both assisted living and memory care at Frank Residences. Plays a key role in staff training for all Health & Wellness staff during orientation and ongoing staff training and skill development. Interacts positively and constructively with residents, families and visitors to address immediate care-related needs or requests and reports to management any situations that require higher level involvement. Supports the administrative functions of the Health & Wellness department for care coordination and delivery, including scheduling appointments, following up with providers and auditing/maintaining clinical documentation.
Essential Job Functions:
- Lead by example with a hands-on approach to create a supportive team approach that consistently uses best practices for the highest quality delivery of care.
- Assist staff to follow all care-related policies & procedures.
- Ensure Resident Service Partners assignments are completed, and report to H&W management teams as issues arise.
- Follow all communication and documentation protocols to ensure excellent service to residents.
- Assist with staff scheduling, as needed.
- Coordinate onboarding and training for non-employee homecare agency staff and ensure files are complete to meet regulatory requirements.
- Provide or assist with in-service training and orientation for all resident service partners, private caregivers and other staff as needed.
- Assist and support RSPs with ADLs and monitor residents for safety.
- Monitor and observe residents for changes of condition and report to management to ensure appropriate care services are delivered.
- Communicate with residents, family, and staff to meet resident needs escalating difficult situations to management for resolution.
- Provide support to Unit Coordinator and Resident Services Manager, as needed.
- Foster teamwork with co-workers by showing initiative in overcoming obstacles, for example, redirecting and reducing challenging resident behaviors in memory care.
- Review daily care logs to ensure all ADLs assignments are met by the care team and report any issues to management.
- Collaborate with Life Enrichment staff and support/assist the RSP team in providing engaging activities at times Life Enrichment staff are not present.
- Document care provided to residents for ADLs, change of condition, and any other event/situation that warrants documentation.
- Follow safety guidelines in the community, including proper lifting techniques and universal precautions when aiding the residents.
- Transfer or escort residents as needed to onsite or offsite appointments or programs.
- Respond to resident emergencies consistent with protocol and training.
- Other duties as assigned.
Requirements
- High school diploma or equivalent.
- Prefer one (1) year experience providing residential care to the elderly or similar experience with specialized dementia care programs.
- Must possess above average human relations, customer service, problem-solving, and organizational skills.
- Must be able to work under time pressures and manage multiple demands simultaneously. Extreme attention to details is essential. Excellent telephone and communication skills are necessary.
- Must be a team player willing to share information and work cooperatively with other members of the management team and caregiving staff.
- Demonstrated high level of interpersonal skills to handle sensitive and confidential situations.
- Demonstrated strong computer skills including MS Office, Excel, Word, and electronic health records.
- Must have the ability to relate professionally and positively and work cooperatively with residents/patients, families, and other employees at all levels.
- Ability to read, write, communicate, and comprehend verbal and written instructions in English.
- Must be capable of maintaining regular attendance.
