Social Worker - Acute Psych

Nestled in San Francisco's Excelsior neighborhood on a nine-acre campus, the San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care. This includes the Frank Residences, offering assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.

Position Overview

Responsible for assessing patients on admission and completes re-assessments at prescribed intervals. Provides individual and group counseling, communicates with families and patient’s authorized representatives, conservators, physicians, unit director and other health team members. Responsible for discharge planning, assisting with advance directives, financial and legal issues and maintains documentation of services in the medical record.

Responsibilities

  • Manages caseload in an efficient manner and coordinates with social workers who are temporarily assigned to the unit re: prioritizing and managing case load.
  • Participates in development, review, revision and implementation of policies and procedures for the Acute Psychiatric Unit as needed and at least annually.
  • Leads and participates in employee education as assigned. Provides instruction and training to patient care and ancillary staff regarding behavioral interventions and psycho social issues.
  • Partners and leads with Unit Director and investigations pertaining to abuse and neglect.
  • Conducts job responsibilities in accordance with standards set out in the Jewish Home’s Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.
  • Clinical responsibilities include: assessments/reassessments, treatment planning, social service counseling, crisis intervention, discharge planning/utilization review, patient teaching, referrals, etc.
  • Performs other duties as assigned.

Qualifications

Education:

  • Master’s degree in social work, required.
  • LCSW, required.
  • Management of Assaultive Behavior (MAB) competency

Experience:

  • Two years experience working with psychiatric patients, preferred.
  • Two years experience discharge planning knowledge or experience in geriatric setting.
  • Experience in coordination of services through the continuum of elder care services.

Pay Range $40.00 - $50.00 DOE
