MDS Coordinator ( Minimum Data Set)
Schedule: Mon-Fri, full time 9am-5:30pm
$59.49/hr to $65/hr DOE

Founded in 1871 as the Hebrew Home for Aged and Disabled and previously referred to as the “Jewish Home”, the San Francisco Campus for Jewish Living (SFCJL) has a long and storied history of providing high-quality care to the community’s most vulnerable older adults. Situated in the city’s Excelsior neighborhood on a state-of-the-art, nine-acre campus, SFCJL includes the 191-unit Frank Residences assisted living and memory care (open since 2020), and the 375-bed Jewish Home and Rehabilitation Center (JHRC), which has the distinction of being the largest, private, nonprofit distinct-part skilled nursing facility in the state of California. Additionally, our campus includes a 12 bed, short-term acute psychiatric unit, unique for its specialization in mental healthcare for older adults. All combined, SFCJL serves a vibrant and diverse population of more than 2,500 patients and residents annually.
Position Overview:
The Minimum Data Set (MDS) Coordinator is responsible for the timely scheduling, completion, and transmission of MDS assessments.
Essential Functions:
- Assure facility compliance with Medicare, OBRA, and Title 22 guidelines
- Support identification of Medicare entitlement and eligibility for prospective residents
- Perform utilization review (UR)
- Oversee gathering of information for assessments and care plans
- Schedule and facilitate care plan conferences
- Participate in committees, including appropriate communication to address and resolves pertinent issues
- Input or supervise the input of MDS assessments, transmission to the state, and obtains receipt of validation
- Manage care planning process
- Complete chart audits as necessary to assess documentation support for MDS assessments
- Identify residents who are having significant change in status, manages interdisciplinary process, and completes necessary assessment
- Monitor MDS documentation and charting requirements that support services provided to meet billing requirements
- Assure timely completion of all required reports and pertinent communication to appropriate staff
- Perform other duties as assigned
Qualifications:
- Graduate of an accredited School of Nursing, RN, required
- Valid California RN license, required
- One year of nursing experience in a long-term care environment, preferred
- Strong knowledge of RAI Process
- Strong knowledge of Medicare regulations – state and federal/CMS guidelines and regulations pertinent to completing, managing, and transmitting MDS assessments
- Able to provide direction and coordination of a multi-faceted program, including problem resolution with facility
- Demonstrated proficiency with MS Office, including Outlook, Word, Excel
- Excellent communications skills, both written and verbal
- Ability to read, write, communicate and comprehend verbal and written instructions in English
- Excellent organizational and interpersonal skills
- Self-motivated and able to work independently, work on multiple tasks in a deadline-oriented environment and ability to be accurate and concise
- Ability to sit and/or work on a computer for long periods
- Ability to communicate effectively with residents and family members, and at all levels of the organization
- Ability to maintain confidentiality
