Nestled in San Francisco's Excelsior neighborhood on a nine-acre campus, the San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care. This includes the Frank Residences, which offer luxurious assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specializing in mental healthcare for older adults (age 55 and over); and the Jewish Home and Senior Living Foundation.

Position Overview:
Reporting to and working closely with the Director of Health & Wellness, the Health & Wellness Manager ensures the quality of care is consistent with established standards, best practices, and in full compliance with all regulatory requirements and internal policies and procedures. The Health & Wellness Manager is a "working manager," responsible for the direct supervision and ongoing support of Health & Wellness (H&W) staff on all shifts, with a primary focus on the direct care staff in assisted living. This role includes providing hands-on assistance with care delivery to set an example and establish standards for all staff. The Health & Wellness Manager assumes clinical leadership and full responsibility for the H&W department during the Director's absence. This position requires the ability to triage and multi-task to ensure completion of assigned tasks while addressing the day-to-day needs of the residents.
Schedule is Tuesday – Saturday.
Essential Functions:
- Ensure direct care staff provide consistently excellent care by training, observing, role-modeling, and participating in direct care delivery.
- Understand RCFE clinical care regulations to ensure regulatory compliance and survey readiness.
- Ensure H&W staff consistently follow clinical care policies and procedures.
- Provide clinical nursing care when needed, including the delivery of medications, injections, wound care, catheter care, and other treatments.
- Conduct frequent audits of the medication program, reporting any errors or concerns immediately to the Director for prompt resolution.
- Ensure clinical documentation is accurate, complete, and up-to-date.
- Ensure continuous staffing coverage.
- Review and approve H&W department timecards in collaboration with the Director of Memory Care.
- Conduct health assessments to pre-screen prospective residents and report to the Director for review.
- Conduct care-plan meetings with residents and families to ensure care plans are up-to-date and that care is being delivered appropriately.
- Ensure annual physician reappraisals are completed in a timely manner.
- Resolve resident and family care-related concerns effectively.
- Prepare internal incident and licensing reports, as well as a monthly tracking log for all falls, hospitalizations, and skin integrity concerns, ensuring all documentation is accurate and current.
- Conduct interviews for new hires and manage the hiring and onboarding process for H&W employees, primarily for assisted living.
- Conduct employee performance reviews in a timely manner at all required intervals.
- Supervise staff on all shifts, providing recognition and partnering with other directors and managers to create an engaged culture for staff, residents, and families.
- Collaborate with other healthcare providers (internal and external) to ensure residents’ healthcare and psychosocial needs are met. Follow up on any outstanding items.
- Evaluate staff knowledge and competency, conduct in-service training, and participate in the orientation of new employees and the ongoing education of assigned staff. Ensure staff are up-to-date with required training.
- Provide coverage for the Health & Wellness Director and Memory Care Director, as needed.
- Ability to work weekends, evenings, and flexible hours to ensure 24/7 delivery of resident care and services.
- Perform other duties as assigned.
Qualifications:
- Current RN or LVN license required.
- Certified in CPR/Basic Life Support.
- Three years of recent nursing experience required.
- Dementia care (Memory Care) expertise preferred.
- Previous supervisory/management experience strongly preferred.
- Assisted living experience preferred.

Pay Range commensurate with experience: $130-140K
