Wellness Nurse Supervisor - LVN/Licensed Vocational Nurse

On-Call, (PM Shift 3 PM - 11:30 PM)

Starting pay at $45/hr (not including potential shift differentials)

Founded in 1871 as the Hebrew Home for Aged and Disabled and previously referred to as the “Jewish Home”, the San Francisco Campus for Jewish Living (SFCJL) has a long and storied history of providing high-quality care to the community’s most vulnerable older adults. Situated in the city’s Excelsior neighborhood on a state-of-the-art, nine-acre campus, SFCJL includes the 191-unit Frank Residences assisted living and memory care (open since 2020), and the 375-bed Jewish Home and Rehabilitation Center (JHRC), which has the distinction of being the largest, private, nonprofit distinct-part skilled nursing facility in the state of California. Additionally, our campus includes a 12 bed, short-term acute psychiatric unit, unique for its specialization in mental healthcare for older adults. All combined, SFCJL serves a vibrant and diverse population of more than 2,500 patients and residents annually.

What you’ll do as a Wellness Nurse Supervisor:

The Wellness Nurse Supervisor ensures that all resident medical records and documents are processed accurately, timely, and filed according to community policies and procedures.

  • Oversee the reporting process/structure for Medication Technicians & Resident Service Partners– ensuring accurate updates/reporting for shift hand-off.
  • Supervise and manage documentation – ensuring accurate, complete, and confidential documentation as it relates to resident medication, treatment, and care records, according to community policies and procedures.
  • Assist the Director of Health & Wellness with receiving and processing resident admissions, readmits, and discharges.
  • Compile new resident medical charts, thin charts into overflow files, and dismantle discharged resident medical charts.
  • Lead communication with physicians, families, and other appropriate parties, concerning resident status, changes in status, need for an appointment, and other health services needs.
  • Responsible for developing and leading ongoing educational trainings and in-services for a wide range of clinical care topics to ensure best practices are followed and consistently maintained by the Med-Techs and Resident Services Providers.

For you to be a successful Wellness Nurse Supervisor:

  • Current and active State of California LVN or RN License.
  • Two years of recent LVN nursing experience is required and/or two years of assisted living experience as a med-tech or caregiver at Frank Residences or comparable care community.
  • CPR/first aid certification is required.
  • Ability to manage and prioritize a complex medication and treatment administration process, within a specified timeframe.
  • Possess sufficient communication and language (orally and written) skills to perform job duties, interpret medical information, and communicate with residents, other staff, family members, etc., as needed.
